Nemaha County Community Health Services will be offering flu shots to students and staff at Sabetha and Wetmore. Marshall Co. will be at Axtell.
FLU VACCINE CLINIC FY22/23
Nemaha County Health Services will be in Sabetha and Wetmore at the following times for the flu vaccine clinic:
SHS: October 11th at 8:30 am
SMS: October 11th at approx. 10:30am (or directly after SHS)
SES: October 13th from 7:30am-9:00am
Wetmore: October 19th from 9:00am-11:30am
For the Sabetha/Wetmore flu clinic please find attached:
1) Flu vaccination consent form
2) Influenza Vaccine Information Statement
3) NCCHS Notice of Privacy Practices.
Please print and fill out both pages of the consent form.
We also ask that a copy of the front & back of each person’s health insurance card(s) be attached to the consent form.
If you do not have health insurance, the cost is $20. A check can be made out to NCCHS.
Please turn in the paperwork in before hand. Having these done ahead of time keeps the flu clinic moving efficiently.
Marshall County Health Department will be giving flu shots from 7:30 AM to 8:45 AM on October 17th at Axtell. Contact Axtell for more information.
